Smith Antibody (Anti-SM)

Specimen Requirement

Collect SST/1.0 mL serum. After 30 minutes of clotting, promptly centrifuge and send refrigerated.

Alternate sample: lavender EDTA. If collecting lavender EDTA, centrifuge, transfer 1.0 mL plasma to a screw top plastic vial, indicate sample type on tube and send refrigerated.

Clinical Use

Anti-Sm antibodies offer a highly specific but relatively insensitive clinical marker of SLE. Their overall prevalence ranges from approximately 20-30% in SLE. Anti-Sm reactivity is not described definitively on other diseases, although a few studies describe Sm antibodies in monoclonal gammopathies and uveitis.
